Instructions for Making a Complaint:

All complaints shall be in writing and signed by the complainant and shall be submitted to the Office Manager and shall be dealt with in the strictest confidence.

Please ensure that the complaint form is completed in its entirety and that handwriting is legible. Failure to do so may jeopardize the process (i.e. if adequate information is not provided, the complaint may not be able to be processed).

Give a description of what has taken place including relevant details as to time, place, etc.
